Guest Information Form
After purchasing the Gala Dinner Guest Ticket, an automated confirmation email will be sent to the email address used during registration.
The confirmation email will include a link to the Gala Dinner Guest Information Form. Please complete the form with the personal details of the guest who will actually attend the Gala dinner, including their full name, email address, and any dietary restrictions or allergies, if applicable.
